Insurance and Billing
We accept most managed care plans. Patient insurance co-payments and deductibles, if any, are collected at the time of service. It is YOUR responsibility to give us the correct information about your insurance plan. It is YOUR responsibility to understand your insurance coverage.
* PLEASE BRING YOUR INSURANCE CARD & PHOTO ID TO EVERY VISIT.
For patients not covered by health insurance, payment in full will be collected at the time of service.
For cosmetic services, which are not covered by insurance plans, payment in full will be collected at the time of the services.
