Your first visit to Musick Dermatology & Advanced Clinical Spa involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more.

There’s also background information about our committed staff and our first visit procedures. You can also save some time by printing out and completing the patient forms in advance of your appointment.

After booking your initial appointment with us, please check your email for the link to access our patient portal. If you did NOT receive a PORTAL LINK, please contact us promptly at (618)628-2588 to have a new activation link sent to you.

Your patient portal allows you the convenience of completing your intake paperwork, request appointments and prescription refills, make payments, view medical records and more.

To minimize check-in time, please upload a copy of your Photo ID and Valid Insurance card(s) below.

We also strongly encourage you to download the “APPatient” app by Modernizing Medicine prior to your visit (see below links). This app will allow you to check in, complete intake forms, and view your appointments on your mobile phone. Please use musickderm.ema.md as the URL for access to your patient portal. If you have any trouble accessing your portal, please contact us promptly to have a new activation link sent to you.
